You are now 41 plus weeks pregnant “postdates”- you are interested in trying to initiate labor on your own.  You should collaborate with your physician/midwife to ensure you have no relative contraindications, (make sure your baby is moving well and no heartrate abnormalities, etc.), you are ready to initiate labor on your own hopefully!

Breast Milk is best!

It’s Baby time! I’m sure you have been informed over and over again, that breast milk is the best!!

Breast milk is the best way to feed your newborn (barring certain medical or physical conditions that limit this). At The Birthing Center of NY, we encourage, teach and support you on how to successfully breastfeed your baby. We take into consideration, your lifestyle, activities, work , social life, home life, medications, surgical and medical history as well as your breast anatomy. We listen to your needs, desire, expectations and your plan for newborn care. For those times when breastfeeding is not possible or not advised, we discuss the proper bottles, milk preparation, etc. with you as well.

Did you know that are various ways to give your baby breast milk . Can you pick out the one that is positively wrong and should not be done at any reason or any time. Good luck! Take my class to learn the answers!!!

Help with breastfeeding

This is day one of your delivery. Your mind and heart has been set on breastfeeding,  You find it is not a good experience.  When you feed the baby you have abdominal pain similar to light contractions.  What should you do? 

  • Stop breastfeeding and give the baby a bottle. 
  • Change your position, you are not properly latching on and its causing you to have abdominal pain. 
  • Take ibuprofen 20- 30 minutes prior to breast feeding – baby is releasing hormone oxytocin causing discomfort. 
  • Obviously breastfeeding is not for you, you should consider bottle feeding,

When do you go to your delivery facility?

You are 41 weeks and scared.  How will you know when it’s time to go to your delivery facility to have your baby.  Do you know? If not, then that is an indication you need to sign up for my class! 

  • when the contractions feels strong and it hurts. You feel pain all around your abdomen and when you walk or change position it’s a little bit better, more comfortable.
  • When contractions are every 5 minutes lasting for at least one minute for one hour and it’s regular.
  • When you have fluid from the vagina and it’s brown or green or it’s not clear or cloudy in appearance 
  • When you have mucus from the vagina with a tinge of bright red or brown streak of blood.

A Question so important I will give you the answer!

Hope you had a safe holiday-this question is so important that at the end I will give the answer- Please, try to figure it out yourself  and if you do not get it right – then you need to take my class!

Question: You are having contractions that appear to be strong and almost intolerable – you call your provider.  What are some signs you might be in active labor or need to be seen for evaluation.

  • You have a large gush of clear fluid from your vagina 
  • You have mucus mixed with a tinge of dark brown or red blood
  • You have cervical changes – your cervix is open up to at least 4-5 cm, and it is getting softer and shorter
  • Your contractions are stronger and harder and more regular intervals, every 3-5 minutes   

Answers:  

  • Your bag of water can break and you may not be in labor, it is expected that within 24 hrs most likely you will go into labor, but you do need to be seen or speak to your provider especially if you are Group B Strep positive on vaginal or urine culture; and if you are preterm (less than 37 weeks); or if you feel the baby is not moving as well
  • “Bloody Show” is the loss of the mucus plug from the cervical canal, indicating the body  is preparing to get ready for the  labor process
  • Labor is defined as regular contractions of the uterus that result in cervical dilation (opening) and effacement (thinning) with descent of the presenting part (hopefully the head)
  • Pain does not necessarily mean you are in labor . Contractions must be strong enough to cause cervical change for you to be in labor! 
  • Sometimes you are in true labor but the baby doesn’t drop due to position of the head, size of the baby in relation to your bones (birth canal)

Are baby hiccups normal?

Your precious baby has hiccups. You wonder if this is normal, should I worry? Hiccups are mostly caused by the irritation to the diaphragm, this is the muscle located at the base of the lungs. When this muscle starts to spasm or cramp, this causes the vocal cords to clamp shut, creating a sound known as hic!!

What do you know about hiccups in newborns?

  • If babies over feed or gulp air during feeding that can cause the stomach to expand and rub against the diaphragm causing hiccups.
  • Feed your baby in a more upright position.
  • Prop your baby head in more of an upright position. Propping head up on a pillow so they are not lying flat may help take less air at mealtime
  • Burp the baby
  • Use a pacifier
  • Use gripe water (Gripe water is an over-the-counter liquid supplement of sodium bicarbonate and herbs (like fennel, ginger, chamomile, dill, lemon balm or peppermint, depending on the formula).
  • If coughing, spitting up, irritable and crying, arching the back especially during or after feeding, seek medical help or call your pediatrician

  • If your partner wants to be intimate  with you,  they are the one responsible for providing protection
  • To ensure safety from sexually transmitted diseases (STD) and pregnancy women should make sure they have the necessary supplies on hand always 
  • Abstinence is the best way to prevent STDs and pregnancy 
  • You should be receiving screening for STD’s and cancer screening from an obstetrician/gynecologist, family medicine physician or other professional with enough training to properly perform examination – and to treat what they may find.    
  • Withdrawal also known as “pulling out” method is safe and effective for not getting pregnant or contracting an STD 

Taking care of yourself and your body is so important. Screening for STD’s may/should include HIV, herpes, syphilis, Hepatitis B and C, gonorrhea/chlamydia and HPV as needed.

What is the best way to sleep during pregnancy?

Now that you are expecting, many questions come to your mind.  You are so unsure about what you can and cannot do.  You ask your obstetrician or midwife- what is the best way for me to sleep? Do you know the best answer?

  • Never use a pillow when you sleep
  • You should alway sleep on your back, it helps with your breathing
  • It does not matter which way you sleep as long as you are comfortable.
  • Because your liver is on the right side of your abdomen, lying on your left side helps keep the uterus off that large organ. Sleeping on the left side also improves circulation to the heart and allows for the best blood flow to the fetus, uterus, and kidneys.

Breast vs, Bottle Feeding – Which is true?

You are breast feeding your newborn and your friend is bottle feeding her baby. Which statements are true and which are false??

  • Always make sure your baby is burped in between bottle or  breast feeds.
  • Breast feeders may not burp because they get less air.
  • Bottle feeders may burp more because they get more air.
  • If a baby pulls away from the breast he/she probably needs to be burped.
  • Feeding the baby too fast with breast or bottle can cause air to accumulate in the stomach.
  • Burping the baby does not release gas and or air from the baby.
  • Bottle feeders should burp baby after 4-5 ounces on milk
  • Breast feeders should burp baby after 2-3 ounces of milk
  • Basically there really isn’t a need to burp the baby after they have finished feeding – that is a myth.

As the weather changes from cold to mild to hot- One may wonder how do I dress my newborn.  Can I use a fan with the baby in the room.  What is the easiest way to tell if my baby is to hot or too cold.  How do I  tell if my baby is too hot?  Your baby may have flushed cheeks. Feel the nape of the neck to see if it feels sweaty or too cold to the touch.  Over heated babies may have sweating and fast breathing. Which statements below are untrue?

·         When dressing a baby the general rule – dress the baby in as many layer as you have on and then add one more layer.

·         Never ever use a fan in the room with a newborn it can cause pneumonia.

·         Babies that sleep in a less ventilated room sleeping with a fan – lower the risk of Sudden Infant Death Syndrome.

Leaving the ceiling fan on a low setting is not going to cause a cold. Baby should be at least swaddled or in a sleeping sack , socks or booties. Most of the newborn heat escapes from the head as well as their tiny little feet.

Make sure your infant is always dry because damp clothing can cause hypothermia.

Use good parenting judgment-  too hot or not too hot?  Consider the temperature outside.
