Baby not breastfeeding – Nurse Jackies Tips for week 1-30-21

Your parents are babysitting your newborn baby so you and your partner can go out for a romantic dinner.  You have been craving Italian food.  You order the spicy penne with Arrabiata sauce. When you come home and you try to breastfeed  your baby,  the baby rejects your milk.  What answer is the best answer.

  • You have emergency stored milk in the freezer. You should pump and dump and give baby the emergency milk
  • Baby is reacting to separation while you were out, do skin to skin and keep trying
  • Call your pediatrician or take baby to an emergency room 
  • Wait and see if the baby will take the breast later

What is the function of the Vernix Caseosa?

When your baby is born,  it probably will have a cheesy white coating called Vernix Caseosa.  If your baby’s vital signs of life such as breathing, color, heart rate, muscle tone, movement, etc. is not good, the baby may need tactile (skin & nerve) stimulation by warming, drying and rubbing the baby’s hands and soles of the feet.  This is done in order to make your baby’s heartrate pick up, get the circulation moving and encourage breathing, this means some or all of the vernix may end up being wiped off the baby to better dry the baby and allowing greater warmth. Can you identify below what is the function of the Vernix Caseosa?

-It can clog the babies pores and prevent the baby for maintaining its temperature.

-Moisturizers the Baby’s skin after birth.

-Helps baby regulate their body temperature.

-It makes the baby look dirty and unclean and should be removed immediately.

-wards off bad bacteria and microorganisms.

-The scent of Vernix triggers neural connection in the brain that is needed for breastfeeding and it helps the baby find the breast on its own.

You are full-term and it is your first child -How should you proceed with labor?

You are full-term and  it is  your first child.  The baby is moving just fine . You are having  contractions that are strong and yet tolerable. Your Birthing Plan is too do this naturally.

How should you proceed with this labor? 

  • Go to your  facility or hospital immediately  to be checked 
  • First baby may take a long time , walk, use a birthing ball, rock/ dance, Breathing  and relaxation exercises, shower  or tub ( tub if not GBS positive and did not break your water bag)
  • Check your bag  for needed item such as insurance card  , ID,  etc.
  • Make sure you eat a hearty meal because your facility or hospital may not allow you to eat    You don’t want to starve !

You are breastfeeding and you find your nipples to be a little cracked ! Nurse Jackies Tip for 7-6-20

What can cause cracked nipples during breast feeding?

Choose the incorrect answer.

  • The baby is not latching on properly
  • You need to reevaluate your sitting position when breast feeding 
  • You are using soap  to wash your breast and that can cause skin cracks in the nipple 
  • It is unknown why the nipples crack,  but it does occur.  This is normal and it is expected to happen.  There is no need to intervene. The nipple will heal!

Can breast milk be stored?

Your plan is to feed your baby Breast milk only. Can Breast Milk be stored ?   Which statement is incorrect ? 

  1. At room temperature (77°F or colder) for up to 4 hours.
  2. In the refrigerator for up to 4 days.
  3. In the freezer for about 6 months is best; up to 12 months is acceptable.
  4. Fresh breast milk or pumped breast milk should be used immediately and never stored .

Correct answer is 4 – You never know what the universe holds for you. Be prepared, have that extra supply of milk your baby may need .

You’re pregnant with a 100.4 temperature – what should you do?

Imaginary Case Study: You’re pregnant and you have a temperature above 100.4 for 2 days and you are coughing. What should you NOT do – there can be more than one incorrect thing to do. PICK THE THINGS YOU SHOULD NOT DO:

1- In case you are admitted to a hospital you should make sure the following people accompany you, Your husband, doula, mother and of course your best friend who is your support.

2-  Do not go to your provider office go to your nearest hospital ER, and immediately let them know you have a fever for 2 days

3-  You should go to your provider office to get immediate care

4-  You should notify your doctor immediately

5 – You should use a mask or  something that looks like a mask, wear gloves

6-  You should make sure you distance yourself from others

7-  Don’t forget to kiss you loves ones before you leave since there is a possibility you may be isolated

8 – Check other household people for signs and symptoms

9 – You do not have difficulty breathing so there is no need for any concern right now

10 –  Hand washing or use of sanitizer is only necessary if you know you touched something

11 –  For prevention of contracting the virus a mask should always be worn in the public whether you are sick or not 

12-  You only need to wear a mask if you are infected or coughing, have a fever or signs of the flu or cold

How do you keep your baby hydrated when you are breastfeeding exclusively?

  • Breast feed emptying the breast completely 
  • Pump and then feed the baby on that breast you pumped for 10 min
  • Feed on each breast for 10-15 min or until your breast is only half empty
  • Let the baby suck on your nipples only

Is your baby dehydrated?

It’s really hot outside.  How can you tell if your baby is dehydrated and needs water?

  • The Fontanel on the top of the head will be sunken.
  • They will make suckling sound.
  • They will suck on their thumb.
  • Their stool will have blood in it.
