Should baby be placed on stomach or chest after birth? Nurse Jackies Tips for 6/1/20

You are finally having your bundle of joy! You have read and your delivery assistant has told you – that when the baby is born it should immediately be placed on your chest for bonding, but your provider places the baby on your stomach and not your chest – Do you know why? Which 2 are the correct answers?

  1. Your provider does not agree with bonding on the chest.
  1. Easier for the provider if the baby is in the abdomen as oppose to  the chest.

3.  Your baby may have a short cord and the cord should not be pulled on otherwise it can pull off and become an emergency

4. Your provider might be focused on something happening vaginally or bleeding from the uterus and needs to pay attention to that – so it was easier to place the baby on the abdomen first.

Lower abdominal and lower back pain during pregnancy – Nurse Jackies tip for 5-23-20

Your were told at 36 weeks you have Group B Streptococcus infection.  Your water bag  just ruptured with a lot of clear fluid leaking.  You need to go to the facility/hospital to give birth. Can you can take a tub bath first before going to the facility?  Which of the following are true statements?

1- Yes you can water will not harm the baby 

2- No, your bag of water to protect the baby is gone and you are introducing GBS directly to the baby 

3- No bath but you can take a shower 

4 No, ideally you should for go to the facility as soon as possible so you can receive intravenous antibiotics for your unborn baby’s wellbeing

Vaginal Exam During Labor Nurse jackies 5-18-20 Tip of Week

You are in active labor for hours and your provider would like to examine you for progress. Which of the following are correct statements? 

1-Vaginal exam is not needed  because it is uncomfortable and it only satisfies the provider of  your labor status 

2-Blood supply and oxygen are decreased with each contraction, if  allowed to continue without intervention to assist labor to progress it can result in poor newborn outcome, such as cerebral palsy .  Vaginal exam would determine  your progress and assess if intervention is needed.

3-vagina exam will identify if the infant is in a favorable position for a vaginal delivery and  that baby has not turned to another position during your labor.

4-vaginal exam will determine if labor is progressing  or if you are taxing the uterus  making the uterus work by contracting  and no progress is occurring 

5-No Vaginal exam is needed because the body is made to deliver, and given time you will deliver  without any intervention.

6-A consent is needed for a provider to do a Vaginal exam (at hospital this is usually found on consent form you sign on admission)

7-No consent is needed for a provider to do a Vaginal exam

Massages During Labor yes or no? Nurse Jackies tip for 5-8-20

You are in active labor you are on your left side. Closing your eyes when there is no contraction and breathing when there is a contraction. Your nurse tells your significant other to put down his phone and massage you!  

What  answer is incorrect?

  • 1- Your  partner does not always need to touch you , you feel better just lying there not being touched and know he is with you
  • 2- You partner needs to massage you as the nurse instructed

Taking Iron During Pregnancy Nurse Jackies 5-2-20 weekly tip

You’re having a baby. Your Obstetrician starts you on iron medication daily for your anemia. What is the recommendation for taking this medication by mouth. Which answer is correct?

  • 1-iron medication needs food to activate it so take it with a meal
  • 2-Iron medication should be taken with milk
  • 3-Iron medication should not be taken with food
  • 4-Iron medication should be taken before you go to bed.

Answer 3 – Iron medications should not be taken with food. The reason you should take iron on an empty stomach without food is that an acidic stomach helps with iron absorption.(unless your physician says otherwise).

Ruptured Membranes – Nurse Jackie’s Tip for 4-24-20

You are going to have a baby. You are sleeping comfortably on your left side. Suddenly you feel some wetness coming from your vagina. Did you pee on yourself or did your water break?

Pick the 2 statements that are accurate regarding Ruptured Membranes..

  • 1 – Amniotic fluid has a distinctive odor 
  • 2 – It’s not near your due date so you could not have broken your water. 
  • 3 – The fluid keep coming out of your vagina.
  • 4 – You are not having any pain  or any other signs of labor, so your water must have not broken .

Answer is 1 & 3 – do not have to be in labor to break your water. It’s called Premature Rupture of Membranes.  You can break your water before you are term it’s called  preterm premature rupture of membranes.   Amniotic fluid has a distinctive smell, different from urine.  Fluid will continuously come from the vagina sometimes, sometimes it will leak for a short time, stop and then do it again.  Bottom line – don’t guess notify your provider immediately because they are a better judge if you broke your water, don’t be surprised if they ask you to be seen either at the office or at the hospital.

My newborn – what part do I not wash?

You just gave birth to a baby with good color, breathing is great, cry is loud and strong and baby is moving about well. Which part of the body should not dried?

  • Head
  • Back
  • Hands
  • Legs

Answer: (3) The hands. Amniotic fluid on the hands help baby find the breast. A babys hands have the scent of amniotic fluid on them and will help your baby find her mother’s nipple, which has a similar scent. For this important reason, your baby’s hands should not be dried and, it’s recommended that you do not wash your nipples before your baby breastfeeds.” This is a beautiful experience to watch!

Can breast milk be stored?

Your plan is to feed your baby Breast milk only. Can Breast Milk be stored ?   Which statement is incorrect ? 

  1. At room temperature (77°F or colder) for up to 4 hours.
  2. In the refrigerator for up to 4 days.
  3. In the freezer for about 6 months is best; up to 12 months is acceptable.
  4. Fresh breast milk or pumped breast milk should be used immediately and never stored .

Correct answer is 4 – You never know what the universe holds for you. Be prepared, have that extra supply of milk your baby may need .

You’re pregnant with a 100.4 temperature – what should you do?

Imaginary Case Study: You’re pregnant and you have a temperature above 100.4 for 2 days and you are coughing. What should you NOT do – there can be more than one incorrect thing to do. PICK THE THINGS YOU SHOULD NOT DO:

1- In case you are admitted to a hospital you should make sure the following people accompany you, Your husband, doula, mother and of course your best friend who is your support.

2-  Do not go to your provider office go to your nearest hospital ER, and immediately let them know you have a fever for 2 days

3-  You should go to your provider office to get immediate care

4-  You should notify your doctor immediately

5 – You should use a mask or  something that looks like a mask, wear gloves

6-  You should make sure you distance yourself from others

7-  Don’t forget to kiss you loves ones before you leave since there is a possibility you may be isolated

8 – Check other household people for signs and symptoms

9 – You do not have difficulty breathing so there is no need for any concern right now

10 –  Hand washing or use of sanitizer is only necessary if you know you touched something

11 –  For prevention of contracting the virus a mask should always be worn in the public whether you are sick or not 

12-  You only need to wear a mask if you are infected or coughing, have a fever or signs of the flu or cold
